Input name

This field is used to collect the user’s name. You can customize both the label and the placeholder to make it more specific or user-friendly. This field is required.

Setting Description

Custom label

The label tells the user what the field is for. It should be short and clear.

Custom placeholder

The placeholder gives a hint or example of what to type. It shouldn’t repeat the instruction; it just shows the expected format.

Option from 1 to 10	This field allows the user to select an issue or topic from a predefined list. You can customize the list of options. Users must select one option from the list. Each option can be edited.
