Contact form section guide
A contact form in a Shopify theme lets customers send messages directly to the store owner. It includes fields like name, email, phone, subject, and message. When submitted, the form sends the details to the store’s admin email.
In the demo store, we’ve created a contact page using a custom page template, divider section and the main contact form section.

How to access the Contact form section
Access this section through the theme customizer:
- Navigate to Online Store > Themes
- Select “Customize” on your active theme
- On any theme template, click “Add section”
- Search and select “Contact form”
To insert a new section between existing ones, follow the steps in the article "Sections and blocks" and locate the section titled “How to add a section”.
Once the “Contact form” section is added, you can customize its appearance and functionality using the available settings.
How to configure the Collection list section
Basic Components
The “Collection list” section consists of two primary components:
- General settings – These control the overall settings of the section.
- Block – Each block has its own individual settings.
It includes blocks - essential input fields for customers:
- Name
- Phone Number
- Subject
- Message
- Submit button
You can further customize this section by adding:
- Side block: you can add address, email, phone of the company
- Text before form
- Text after form
Also Custom liquid block can be added.
